Applicant must have a 3.2 GPA to be eligible for this award, but at least as much consideration will be given to student's athletic achievement as is given to academic achievement. Signified the intention to continue academic work beyond the high school or prep school level. |

The application to be completed and signed by the nominee - must be printed or typed. The nomination and endorsement form to be completed and signed by the faculty athletic representative or the director of athletics.
An evaluation form to be completed and signed by the head hockey coach. An endorsement to be completed and signed by the applicant’s academic advisor.
